  1. Valid passport last 6 months,
  2. Schengen visa application form,
  3. Three color photographs 35mm by 45mm 80% of the face should be seen white background,
  4. the original invitation from the party was verified by the Czech police and has to be in the Czech language,
  5. Overseas medical (with Original payment receipt of Purchase of Overseas medical policy with all visa application),
  6. Personal Income tax papers for last 3 years
  7. Company Income tax papers for last 3 years
  8. Personal Bank Statement of last 6 months.
  9. Company Bank Statement of last 6 months.
  10. Business correspondence Credit Card or Forex,
  11. Covering letter on company letterhead,
  12. Return confirm Ticket

TOURIST VISA

  1. Valid passport for 3 months from the date of return with 02 pages back to back blank
  2. Two-color photos 35mm by 45mm white background 80% of the face should be seen.
  3. Covering letter from pax on company letterhead with company round stamp on it.
  4. If working then covering letter on plain letter & Original Leave letter from the Indian company.
  5. Confirmed Hotel confirmation as per the air tk
  6. Personal & Company bank statement copy for last 06 months of the passenger with stamp & signature in all the pages.
  7. If working Salary Slips for last 3 months
  8. Personal & Company I T papers copy for last 03 yrs of passenger
  9. Listed Overseas medical insurance as per the Air tkt List of Insurance is mentioned below
  10. Return Air ticket
  11. Listed Overseas medical insurance as per the Air tkt List of Insurance is mentioned below
  12. Company registration copy
